In addition to diligent research to prove out the idea from the start, we assembled a leadership team and advisory board beyond compare, dedicated to engaging with a patient’s community as well as insurers and facility partners to support overall health.

Dr. Dave Moen is a thought leader in geriatric services and accountable care with long-standing connections to national Family Medicine Academic Training programs. Prior to TeamMD, he served as CEO of Fairview Physician Associates, a leading Accountable Care Organization in Minneapolis, Minn. He is former President of Bluestone Solutions and a former member of Fairview Partners Governing Council — organizations committed to best-in-class geriatric health services.

Dr. Nick Schneeman is the founder of Genevive, a company that since 2004 has designed a team-based approach to geriatric healthcare and care management. He created a practice of 115 physicians, NPs, RNs and social workers that serves the dual-eligible Long-Term Services and Supports (LTSS) population in Minnesota. Dr. Schneeman is advancing the care protocols of this successful practice for TeamMD. 

Bror Hultgren is a recognized innovator in the areas of Complex Population Programs, Medicaid Managed Care and Long-Term Care, and Dual and Institutional Special Needs Plans. He began his career in social case work and has extensive experience in behavioral health and case management. Bror is former CEO of UnitedHealth Group’s Community & State Central Region and SVP of Complex Population Programs. He holds an MBA and MS in Health Administration from the University of Colorado Denver.

Jon Redmond brings TeamMD extensive operational experience from his vast career in private industry and service as a captain in the U.S. Army. He earned his Bachelor’s of Science degree from West Point and holds advanced degrees from the University of Oklahoma and Harvard Business School.

Build a physician-quarterback team for available, on-demand, high-quality care while keeping patients out of the hospital and living independently.

Age with Honor

Specially trained TeamMD physicians coordinate with nurse practitioners, hospital personnel, specialists and other care team members to eliminate communication gaps and provide personally managed care.

 A customized care plan allows patients and their families to participate in planning, and also engages local nursing homes and assisted living in effective partnerships.

This proactive, synthesized approach prevents uninformed care and avoids emergency measures. As a result, patients receive the highest level of care, and are truly able to age with honor.

We developed a specialized, technology-enabled coordination of caregivers, with Community-Based Case Managers functioning as a primary communication conduit, documenting and coordinating every step.

TeamMD creates the relationships and communication plans that enable seamless care experiences. This includes partnering the patient, their family, and care teams with local healthcare plans, nursing homes and assisted living facilities.

Structured processes are in place to minimize unneeded and potentially harmful procedures and medications.

This approach invests resources where they will make the greatest impact, and minimizes the costs of unnecessary procedures.
